#277 Regen rewrite

- need to consolidate regen calcs - support in and out of combat - rule for classic that only gives in or out of combat individually — after dof out of combat gets both out and in combat regen - need a way to track player being engaged in any active encounter (vs moving in and out of 'combat' stance)
