#85 server->client desync on AppCombined packets based on size/count

In LE_DEBUG there is huge desync due to these settings. This extends to the server->client relationship outside of LE_DEBUG also, just not as prevalent.

This is split from the issue http://cutpon.com:3000/devn00b/EQ2EMu/issues/61

client hard crashes on logging into zone when this issue occurs. It is not a kick back to character select like what happens in #61.
